If you wish to use your insurance for your counseling needs, the following is standard information you need to gather from your insurance company prior to your first visit with Pathways Pastoral Counseling:

1) Call the 1-800 number on the back of your card and find out if you have mental/behavioral health coverage on your policy.

2) Ask if you have a deductible that needs to be met first and if so how much is that and how much has already been used.

3) If your policy does not require a deductible to be met, is there a copay and/or coinsurance payment and get them to tell you the amount.

4) Do you require a pre-authorization number prior to counseling services? If so, give them the name Rupert E. Kuhne and Pathways Pastoral Counseling at 400 Dupre Dr., Spartanburg, SC 29307. They will find me in their system and supply you with a pre-authorization number.

5) Please bring this information with you to your first session.
